Our Commitment to Your Privacy

Bare Medispa ("we," "us," or "our"), located in Bismarck, North Dakota, is dedicated to maintaining the privacy of your protected health information (PHI). PHI is information that may identify you and that relates to your past, present, or future physical or mental health or condition and related health care services. This Notice of Privacy Practices outlines our legal duties and privacy practices concerning your PHI. We are required by law, including the U.S. Health Insurance Portability and Accountability Act (HIPAA), to maintain the privacy of your PHI and to provide you with this notice.

This notice applies to all information and records related to your care that we maintain at our Bismarck facility.

This includes using your PHI to provide you with aesthetic and medical spa treatments and services, and disclosing it to doctors, nurses, technicians, or other personnel involved in your care. Furthermore, we may use and disclose your PHI for our health care operations, which are necessary to run our facility and ensure quality care for all clients, such as for quality assessment, employee reviews, and training. We may also use your information to send you appointment reminders or to inform you about treatment alternatives and other health-related benefits. Finally, we will disclose your PHI when required by federal, state, or local law, such as in response to a court order. We may also disclose PHI to public health or governmental authorities for public safety matters or if we believe a patient has been the victim of abuse or neglect.

Your Rights Regarding Your Protected Health Information

You have several rights regarding the PHI we maintain about you. You have the right to inspect and copy your PHI by submitting a written request to our Privacy Officer. If you believe the information we have is incorrect or incomplete, you have the right to ask us to amend it by providing a written request with a supporting reason. You also have the right to request an accounting of certain disclosures we have made of your PHI for purposes other than treatment, payment, or health care operations.

Additionally, you have the right to request a restriction on how we use or disclose your PHI, though we are not required to agree to your request, we will try to accommodate it. You may also request that we communicate with you about medical matters in a certain way or at a specific location. Lastly, you have the right to receive a paper copy of this notice at any time upon request.

Changes to This Notice

We reserve the right to change our privacy practices and to make the new provisions effective for all PHI we maintain. Should our privacy practices change, we will post a copy of the revised notice in our facility and on our website, https://www.baremedispa.com/
